Drew McIntyre was the last person to be tossed out of the ring by Bock Lesnar after the latter entered the Royal Rumble at number 30. Lesnar hit McIntyre with an F5 outside the ring to score the final elimination.

According to Fightful Select, WWE weren’t sure they could bring back McIntyre in time for the rumble due to recurring neck issues. The former WWE champion has been working with an injured neck since 2016.

The outlet said, “Drew McIntyre’s appearance was kept very quiet, and him even being able to appear wasn’t a sure thing. We’ve been told that he’s not quite 100 percent soon, and is continuing to rehabilitate his neck.”

“He first sustained his injury in 2016, and has been working around it ever since. There were many within the company that had no idea Drew McIntyre was back until he appeared at Gorilla position.”

Dave Meltzer recently noted that WWE had plans for McIntyre and Lesnar to perform more spots in the Men’s Royal Rumble, but ended up scrapping those plans altogether due to time constraints.
